You can't tell if a project is failing or if performance is improving if you don't set clear goals and have objective measurements. Avoid misleading percentages and red/yellow/green coloring exercises. What gets measured gets managed. Know the actual numbers and the productivity of every member of your team. Track failures and problems in a database. Provide feedback, set expectations, measure improvements, and look for ways to build on success.
